Before a damaged roof ever becomes a filed claim, something else is already happening. Public adjusters are buying search ads. Doppelganger websites are going live. Lead generators are positioning themselves to intercept confused policyholders the moment they turn to a search engine for help and steer them to litigate instead.
In this episode of the Predict & Prevent podcast, Pete Miller, CPCU, CEO of The Institutes, sits down with Todd Kozikowski, CEO of 4WARN, to explore a hidden and rapidly growing blind spot in the insurance industry: the digital activity that occurs before a policyholder ever files a claim.
Kozikowski explained how bad actors — from lead generators and referral networks to overseas cyber criminals — use search advertising, doppelganger websites, and AI manipulation to intercept and influence claimants at their most vulnerable moment.
“Tech-enabled claim instigation” is the term 4WARN uses to describe this phenomenon. It refers to the coordinated use of digital tools designed to intercept confused or frightened policyholders the moment they turn to a search engine for help.
“After a tree falls on the roof of a home, most of us don’t have our insurance company’s claims number saved in our phone,” Kozikowski said. “And right there is where the journey begins, because what you get may not be the insurer’s information.”
What started as an investigation into why litigated claims in Florida were outpacing those insurers’ market share has grown into a national intelligence platform monitoring trillions of digital signals across thousands of actors and companies every day. 4WARN’s approach is distinctly outside-in — rather than analyzing claims files after the fact, the company watches the digital environment before a claim is ever filed, identifying threats as they form.
The results have been striking. Louisiana Citizens, the state’s insurer of last resort, reduced its litigation rate by one percent in just three months — a savings of nearly $9 million. PCH Mutual, which provides insurance to senior living facilities, cut its litigation rate by more than 40% and reduced online targeting of its brand by over 50% in a similarly short window.
The threats Kozikowski describes go far beyond aggressive attorney billboards. He details how fraudulent websites have been hosted on servers in Norway, how Eastern European cybercriminal organizations have targeted U.S. carriers to harvest policyholder data for the dark web, and how bad actors are now actively corrupting AI-generated answers to search queries to steer claimants away from their insurers.
His advice to any insurance executive listening is immediate and actionable: search for your own company the way an anxious policyholder would right after a loss. “The goal,” he said, “is to make sure someone else is not quietly defining your company’s claims journey for its policyholders.”
