Chad Schulz stands up for the small companies — and wins. He recognizes the challenge that fully-insured smaller employers face with limited strategic options to address the rising cost of health care. In a fully insured world, these options are often limited to cost shifting, carrier changes, or reducing the underlying benefit plan design.
Given the rigidness of fully insured carriers, smaller employers are often unable to take advantage of some of the innovative strategies larger employers have been deploying for years. One of his clients faced this challenge after consistently receiving double-digit initial increases in rates.
This resulted in significant disruption to the employees in the form of premium increases, decreased benefits, and changing carrier networks, contributing to the 25% attrition rate the client had. Schulz analyzed the current benefits package and created a plan.
His changes included: self-funding the medical plan with a TPA, utilizing a stop-loss captive, adding concierge medical services to employees, integrating with vendor solutions for managing major chronic medical conditions, and incorporating steerage to only high-quality physicians and facilities.
This resulted in a three-year savings of $1.6 million. In addition to reducing costs for the client, the employer was able to significantly enhance the benefit plan by offering a lower deductible and made both plans more affordable with lower premiums — significant benefits for employees that may help lower attrition rates.
